Birth and Death Certificates

Missouri Births
The Webster County Health Unit can provide certified copies of Missouri birth certificates for $15.00 each. Birth certificates are available for births from 1920 to present. Births before 1920 may be obtained through the Missouri Department of Health and Senior Services (DHSS) by calling (573) 751-6387, or by going online to www.vitalcheck.com.

Birth certificates will only be issued to the registrant, relatives up to, but not including cousins, a legal guardian of the child, a step-parent or other legally authorized individual as determined by Missouri Department of Health rules relating to access to vital records.

Average wait time to receive a birth certificate is 5 - 10 minutes. Please be prepared to provide the following:
  • registrant's full name at birth
  • registrant's date of birth
  • registrant's place of birth
  • mother's full maiden name
  • father's full name
  • photo identification for person requesting birth certificate

Out-of-State Birth Certificates
Certificates for births that took place outside the state of Missouri can be obtained by visiting www.vitalchek.com or the National Center for Health Statistics. Fees and delivery times may vary by state.

Missouri Deaths
Certified copies of Missouri death certificates will be provided to qualified individuals for $13.00 for the first copy and $10.00 for each subsequent copy in the same visit.

Death certificates are available for deaths occurring from 1980 to the present. For deaths occurring before 1980, contact the Missouri Department of Health and Senior Services (DHSS) by calling (573) 751-6387, or by going online to www.vitalchek.com.

Death certificates may be issued to relatives or to third parties with a legal need. Please be ready to provide the deceased person's full name and date of death. If you are a third party, you may also be asked to fill out additional paperwork in order to receive the death certificate.
